St. Mary's Innovis launches Eldercare
St. Mary's Innovis Health announced the start of its Eldercare program
The Eldercare program works with a health plan to provide care coordination and medical oversight to those who are eligible for both Medicare and Medicaid services, live in the region, and who have an Innovis physician for primary care services.
The program serves the areas of Detroit Lakes Moorhead, Park Rapids, Menahga and Fosston.
Dr. William Henke is the medical director of the Eldercare program and will provide medical oversight of enrollees, including personal visits and an annual care conference. He will also work with primary care physicians to coordinate services.
Henke obtained his medical degree from the University of Minnesota, Minneapolis in 1979.
He has been the medical director at Emmanuel Community since 1983, the current medical director of Oak Crossing and is a Regional Medical Director for Hospice of the Red River Valley in Detroit Lakes.
Ashley Kolstad has joined the St. Mary's Innovis Health Eldercare program as a Care Coordinator.
Kolstad attended Minnesota State Community and Technical College where she earned her LPN degree in 2007 and RN degree in 2008.
She has been employed at St. Mary's since 2005, most recently on the Medical-Surgical/Pediatric unit.
Kim Schossow has joined the Eldercare program as Care Coordinator.
Schossow has been employed at St. Mary's Innovis Health since 1997, working on the Medical-Surgical/Pediatrics Unit for 11 years and most recently on the Obstetrics Unit.
For the past 6 years, Schossow has been the childbirth educator.
She received her RN degree from Normandale Community College in Bloomington and is currently attending Minnesota State University, Moorhead to obtain her Bachelor of Science in Nursing.
